When finished, read these two forms carefully, fill them out, sign them and submit them to kcoakley (at) ecparamedics.com. If you're under 18 years old, you'll need a parent or guardian's signature, too.
When we receive your paperwork, we'll reach out to schedule the ridealong. Depending on which station you're assigned to (we have five stations in Vail, Avon, Edwards, Eagle and Gypsum), you'll report between 8 a.m. and 9 a.m. You can ride along for 8-10 hours. If you have a preference for station, please let us know and we'll try to accomodate that request.

Eagle County Paramedic Services is fully committed to providing accessible facilities, elements and
channels of communication to all members of the public. As part of this commitment, Eagle County Paramedic Services
