What is Eagle Valley Home Health?
Eagle Valley Home Health (EVHH) is a service from Eagle County Paramedic Services that provides skilled nursing needs for patients that qualify for home health. Currently, EVHH utilizes one home health nurse; we are hoping to engage a part time physical therapist in the near future. Seeking to expand the ability to treat patients in the home, EVHH provides care that supplements our community paramedic program, bridging the healthcare gap for our community.
What is skilled home health?
Skilled home health is physician-directed care provided by licensed clinicians (RN, PT) in a patient's home to treat illness, injury, or manage complex conditions. Services are covered under Medicare/medicaid and most insurances when eligibility criteria are met.
Our Goals:
- Promote recovery – Support patients after illness, injury, or surgery with skilled nursing, therapy, and education.
- Prevent complications – Monitor health status and intervene early to avoid ER visits or hospital readmissions.
- Manage chronic conditions – Help patients live safely with conditions like heart failure, COPD, or diabetes through ongoing assessment and medication management.
- Support independence – Teach patients and caregivers how to manage care themselves.
- Improve quality of life – Provide care in the comfort of home, reducing stress and preserving dignity.
Who Might Be a Home Health Patient?
A home health patient may be:
- Recovering from surgery or hospitalization (e.g., joint replacement, cardiac procedure)
- Managing a chronic illness (e.g., COPD, heart failure, diabetes)
- In need of skilled nursing for wound care, catheter or ostomy management, or IV medications
- Having difficulty leaving home due to weakness, pain or disability
- At risk of rehospitalization and needing close monitoring at home
- Requiring physical therapy to regain independence
- Needing medication education or management after a new diagnosis or medication change
- Experiencing frequent falls or changes in mobility
- Note: Medicare patients must be homebound or require substantial support to leave their home.
