Position Overview:

The Med Pool Community Health Nurse will provide compassionate, high-quality care in the comfort of patients’ homes that empowers individuals to live healthier, more active lives in the beautiful outdoors of Eagle County, Colorado. The Med Pool Community Health Nurse will have the opportunity to develop and implement individualized care plans, provide chronic disease management, work alongside EMS providers to respond to acute health issues in the home setting, teach patients and their families about managing various health conditions and have a lasting impact on improving the quality of life for individuals in our community. One of the greatest aspects of this Med Pool Community Health Nurse position is the experience of working with a skilled, dynamic team of healthcare professionals from ECPS Community Health. We play a pivotal role in providing innovative solutions for each patient’s health journey.
